胃酸过多、反胃、恶心呕吐:为什么很多人的胃病反复多年?
在美国,很多人长期受到胃部疾病的困扰。胃胀、反酸、嗳气、恶心、呕吐,让许多人不敢吃自己喜欢的食物。有些患者甚至到了“吃什么都不舒服”的程度,生活质量受到很大影响。
我最近治疗的一位患者,胃部不适已经持续了十年。一个普通的小肉丸,对别人来说很平常,但对他来说,吃下去以后就会出现恶心、呕吐、打嗝、胃胀、消化不良等症状。长期以来,他只能小心翼翼地选择食物,害怕吃错东西引发胃部不适。
那么,为什么会出现这些问题?
胃酸到底是好还是坏?
很多人认为胃酸是不好的东西,其实并不是。
正常情况下,胃酸是人体消化系统不可缺少的一部分。胃需要胃酸帮助消化食物,同时保护身体免受部分有害微生物影响。
但是,当胃酸分泌过多,或者胃部动力下降、胃内容物反流到食管时,就可能出现胃食管反流病(GERD,Gastroesophageal Reflux Disease)。
常见表现包括:
胃酸反流、烧心
反胃、恶心
打嗝、胃胀
胃部不适、消化缓慢
夜间躺下时症状加重
为什么会发生胃酸过多和反流?
胃食管反流和胃酸异常,通常与多种因素有关:
1. 饮食因素
长期饮食不规律,过量食用刺激性食物、油腻食物、过甜食物,或者进食过快,都可能增加胃部负担。
2. 情绪压力
现代生活压力大,长期焦虑、紧张、睡眠不足,会影响胃肠功能。中医认为,情绪变化可以影响肝气运行,进而影响胃的功能。
3. 胃部疾病因素
例如幽门螺旋杆菌感染、胃黏膜损伤、胃动力异常等,都可能导致胃部长期不适。
同样是胃酸反流,为什么每个人治疗方法不同?
很多患者问:“胃酸多,是不是都用同一种方法治疗?”
答案是否定的。
从中医角度看,胃病不能只看一个症状,而要根据个人体质、寒热虚实进行辨证。
有些患者属于:
肝郁犯胃
情绪压力大,容易生气、紧张,同时出现胃胀、反酸、嗳气等表现。
中医治疗会考虑疏肝和胃、调理气机,例如根据辨证选用相关方剂。有些肝胃不和、胃热表现明显的患者,临床上可能会参考左金丸,一贯煎类方的思路进行调理,但具体使用需要根据个人情况判断。
左金丸:黄连,吴茱萸
胃寒体质
有些人平时怕冷,喜欢热饮,吃寒凉食物后容易胃痛、胃胀、不舒服。
治疗方向通常偏向温胃散寒、改善胃的运化功能。参考吴茱萸干姜汤等。
胃热体质
有些患者表现为口苦、口干、胃部灼热感、反酸明显等,需要根据热象进行调理。参考旋覆花代赭石汤等。

胃阴不足
部分长期胃病患者,由于久病耗伤胃阴,可能出现胃部不适、干燥、隐痛等表现,需要从养阴和胃方面考虑。参考一贯煎,养胃增液汤等。
治疗胃病,关键是找到自己的原因
胃酸反流只是一个表现,背后的原因可能不同。
同样是“反酸”,有人是寒,有人是热;有人与压力有关,有人与胃动力有关;有人需要调理胃气,有人需要改善饮食和生活习惯。
因此,治疗胃病不能只针对一个症状,而应该结合:
个人体质
饮食习惯
情绪状态
睡眠情况
胃部检查结果
进行综合判断。
胃是人体重要的消化器官,保护胃健康,需要长期调整,而不是只在症状出现时临时处理。

Acid Reflux, Nausea, and Vomiting: Why Do So Many People Struggle with Stomach Problems?
In the United States, millions of people suffer from chronic digestive problems. Acid reflux, bloating, belching, nausea, and vomiting can make everyday eating a challenge. Many people avoid foods they enjoy because they are afraid of triggering uncomfortable symptoms.
Recently, I treated a patient who had struggled with digestive problems for over 10 years. Even eating a small meatball would trigger nausea, vomiting, frequent belching, bloating, and poor digestion. For years, this patient had to be extremely careful about what to eat because almost any mistake could lead to significant discomfort.
Is Stomach Acid Good or Bad?
Many people think stomach acid is harmful, but that is not entirely true.
Under normal circumstances, stomach acid is essential for healthy digestion. It helps break down food and serves as one of the body's natural defenses against harmful microorganisms.
However, when stomach acid is produced in excess, or when the lower esophageal sphincter does not function properly, stomach contents can flow back into the esophagus. This condition is commonly known as Gastroesophageal Reflux Disease (GERD).
Common symptoms include:
Acid reflux or heartburn
Nausea
Regurgitation
Frequent belching
Bloating
Slow digestion
Symptoms that worsen when lying down
What Causes Acid Reflux and Excess Stomach Acid?
GERD and acid-related digestive symptoms are often caused by multiple factors, including:
1. Dietary Habits
Irregular meals, overeating, spicy foods, fatty foods, excessive caffeine, alcohol, and eating too quickly can all place extra stress on the digestive system.
2. Stress and Emotional Factors
Chronic stress, anxiety, and poor sleep can affect normal digestive function. In Traditional Chinese Medicine (TCM), emotional stress is believed to disrupt the smooth flow of Liver Qi, which may affect the stomach's ability to function properly.
3. Underlying Digestive Disorders
Conditions such as Helicobacter pylori infection, inflammation of the stomach lining, delayed gastric emptying, or other gastrointestinal disorders may also contribute to chronic digestive symptoms.
Why Doesn't Everyone Receive the Same Treatment?
Many patients ask,
"If I have acid reflux, shouldn't everyone receive the same treatment?"
The answer is no.
From a Traditional Chinese Medicine perspective, patients with the same Western diagnosis may have very different underlying patterns of imbalance. Treatment is based on an individual's constitution and pattern of symptoms rather than the diagnosis alone.
Some common TCM patterns include:
Liver Qi Invading the Stomach
Patients may experience symptoms that worsen with stress or emotional tension, including acid reflux, belching, bloating, and stomach discomfort.
Treatment focuses on regulating Liver Qi and harmonizing the stomach. In patients whose symptoms fit this pattern—particularly when heat signs are also present—a practitioner may consider formulas such as Zuo Jin Wan (Left Metal Pill) as part of an individualized treatment plan.
Stomach Cold Pattern
People with this pattern often feel cold easily, prefer warm drinks, and experience stomach discomfort after consuming cold foods.
Treatment generally focuses on warming the stomach and improving digestive function.
Stomach Heat Pattern
Patients may experience burning sensations in the stomach, excessive acid reflux, thirst, bad breath, or irritability.
Treatment aims to clear excess heat while restoring normal stomach function.
Stomach Yin Deficiency
People with longstanding digestive disorders may gradually develop a deficiency of stomach yin, resulting in chronic stomach discomfort, dryness, or a burning sensation.
Treatment focuses on nourishing stomach yin and restoring balance.
The Key to Successful Treatment Is Identifying the Root Cause
Acid reflux is a symptom—not the root cause.
Two patients may both have GERD, but one may have a cold pattern, another a heat pattern, one may be primarily affected by emotional stress, while another may have impaired digestive motility.
For this reason, treatment should be individualized and may include a comprehensive evaluation of:
Overall body constitution
Eating habits
Emotional health
Sleep quality
Medical history and diagnostic findings
Both Western medicine and Traditional Chinese Medicine recognize that chronic digestive disorders often require a comprehensive and long-term management plan rather than simply treating symptoms as they arise.
